- Spouse: 1 photograph — optional, if spouse is registered.
- Children: 1 passport-size photograph — optional and 1 Aadhaar card image (front) per child — required if adding the child to the membership.
- Accepted image formats: JPG or PNG.
- Maximum file size: 1 MB per file

The attached form asks for the following information:
- Membership number.
- Member name, mobile number and address.
- Email address and occupation, where applicable.
- Spouse name and mobile number, if applicable.
- For each child: name, age, gender , Aadhaar Card number and Aadhaar card image (front) in JPG or PNG format.
